Hospice Admission RN – Job Summary

2pm-11pm afternoon shift

– $10,000 Sign-On Bonus

As a Hospice Registered Nurse (RN), you will apply critical thinking and clinical expertise to perform comprehensive assessments, develop personalized care plans, and deliver high-quality, compassionate care. Your goal is to maximize the comfort, dignity, and well-being of patients and their families in alignment with the interdisciplinary plan of care and all regulatory standards. In the role of Hospice Admission RN, you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.

 What You’ll Do

  • Provide skilled nursing care in accordance with physician orders and organizational policies
  • Perform comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, therapists, social workers, and home health aides
  • Supervise and support LPNs and Home Health Aides
  • Ensure compliance with regulatory standards and contribute to quality improvement initiatives
  • Promote patient satisfaction and positive health outcomes

What You Bring

  • Current RN license (BSN preferred)
  • Minimum 1 year of nursing experience (required in California)
  • Home healthcare experience preferred
  • Valid driver’s license and reliable transportation
  • Strong computer skills and a commitment to ethical, mission-driven care

 Physical & Mental Requirements

  • Frequently lift, stand, and walk (35% of time)
  • Occasionally bend, stoop, twist, and assist with patient positioning (10%)
  • Independently lift up to 35 lbs
  • Perform detailed tasks with frequent interruptions and shifting priorities
  • Adapt to changing work environments and travel to various work locations
