Licensed Practical Nurse (LPN)
Location: LIFE at New Jersey– Trinity Health PACE
Job Type: Full-Time

Position Purpose
Join our mission-driven team at LIFE at New Jersey, where you’ll provide skilled nursing care and oversee service delivery to participants in our PACE program. You’ll play a vital role in supporting the health, independence, and dignity of older adults.

What You Will Do

  • Deliver nursing care and prescribed treatments per professional standards and primary care orders
  • Collaborate with interdisciplinary teams to develop and implement care plans
  • Educate participants and families on treatments, medications, and self-care
  • Maintain accurate documentation and participate in quality improvement initiatives
  • Support participants across center, clinic, and home settings
  • Participate in on-call coverage and disaster recovery planning

Minimum Qualifications

  • Graduate of an accredited nursing program
  • Active LPN license in the state of the PACE organization
  • CPR certification required
  • 2+ years of experience with geriatric populations preferred
  • Experience with frail or elderly individuals or willingness to complete training
