Trinity Health at home is seeking a home care Scheduler located close to a Trinity Health office for the Athens, Ga. area.
Responsible for scheduling clients Home Health/Hospice services with clinician continuity. Identifies client need and schedule clinicians with the appropriate competencies to match client level of care. The Scheduler is expected to exercise judgement in determining the most effective and efficient method of scheduling and rescheduling patients. Provide optimal levels of communication with patients, clinicians, clinical managers, physicians, and homecare coordinators. May also provide coverage for multiple sites and/or Hospice support.
Must be able to work:
Monday through Friday 8:00 am -4:30 pm
1 weekend a month
Minimum Qualifications
High School diploma required with equivalent 2-4 years industry experience; Associate's Degree preferred
2-3 years home care, hospice experience preferred
Microsoft computer skill set required; experience with Trinity Health At Home software platforms and applications preferred
Working knowledge of standard office equipment and remote technology required
Experience multi-tasking on multiple computer systems..
Epic experience a plus
Must have valid Driver’s license and reliable transportation to and from work site.
