POSITION PURPOSE:
As a Case Manager within our Community Treatment Team, you will be responsible for providing comprehensive case management services to individuals with mental health and behavioral health needs within the community as well as a combination of office-based work. You will work closely with clients, their families, healthcare providers, and other community resources to ensure they receive appropriate and timely support and services.
WHAT YOU WILL DO:
MINIMUM QUALIFICATIONS:
• Bachelors in Human Service such as; sociology, social work, psychology, criminal justice, gerontology, counseling, education or nursing and (3) years of experience in a related field OR Masters in Human Services such as; sociology, social work, psychology, criminal justice, gerontology, counseling, education or nursing and1 year of experience in a related field.
• Act 33/34/73 clearances.
• Pre-employment Drug Screen
• Valid driver’s license
• Ability to drive an organization vehicle
POSITION HIGHIGHTS/BENEFITS
WORK SCHEDULE: This is an Exempt (salary) position. Monday through Friday daylight with flexibility to meet consumers’ needs. Nontraditional hours (evenings, weekends, and holidays) may be required. Will provide on call services on a rotating basis ($50.00 flat rate daily)
