Center Manager – Trinity Health PACE
Location: St Joseph PACE
Job Type: Full-Time

Position Purpose
As Center Manager, you will lead the daily operations of the PACE center, ensuring high-quality, coordinated care for older adults. You’ll oversee interdisciplinary team members and manage services including rehabilitation, social work, dietary, and recreation.

What You Will Do

• Supervise and support IDT staff across multiple service areas
• Facilitate care planning and team collaboration for participant needs
• Manage center operations, budgets, and regulatory compliance
• Lead quality improvement initiatives and survey preparedness
• Participate in strategic planning, staffing, and technology integration
• Serve as a key liaison with clinical leaders and senior management

Minimum Qualifications

• Bachelor’s degree in healthcare or related field
• 5+ years of supervisory or administrative experience in healthcare
• 1+ year working with frail or elderly populations (or willingness to train)
• Strong leadership, communication, and organizational skills
• Ability to work independently and collaboratively in a dynamic environment
