Description:

Position Summary: The Case Management Coordinator works under direction of the Case Management Lead and is an integral part of the Case Management Department. The CM Coordinator is a skilled professional who provides the complete administrative support duties and responsibilities for the Case Management Department. The CM Coordinator works closely with CM team on special projects and other duties as assigned by the lead. The CM Coordinator must have an in-depth knowledge of and ability for preparation of recurring and special reports of clinical metrics for dashboards and activities, such as statistical reports for operational and performance improvement.

Position Requirements:

Must exercise a high degree of initiative and independent decision-making qualities in carrying out duties.  Attention to detail and high degree of accuracy required, as well as integrity and respect involving all matters of a confidential nature.

Knowledge:  Must have a working knowledge of Hospital and department personnel policies and procedures.  Should have a general understanding of admissions, management information systems, medical records, insurance, and patient billing process.  Knowledge of medical terminology necessary.  Knowledge of payer sources and managed care contract terms desirable.

Essential Functions and Responsibilities

Coordinate the communication of incoming information- telephone calls, voicemails, fax, mail, etc Assist with coordination of URC meetings by gathering key statistics and preparing reports. Issue Medicare Important Message Letters and perform monthly audits for compliance. Establish and maintain collaborative and effective working relationships with internal and external customers.    Document in a Trinity approved Electronic Medical Record -EMR. Assist with completion of referrals to post-acute service providers. Maintain a working knowledge of applicable Federal, State and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical and professional behavior. All administrative duties.

OTHER FUNCTIONS AND RESPONSIBILITIES

Performs other duties as assigned.

Required Education, Experience and Certification/Licensure

Education:     Associate degree in business or healthcare, Project Management, or related field; or an equivalent combination of education and experience. Advanced computer skills. Bachelor’s Degree preferred.

Experience:   Require- 3-4 years in administrative/secretarial experience and 1-2 years of in health-related field.
