What you will do:
The Case worker will join the multidisciplinary Samaritan PROS team which is part of the Samaritan Hospital’s Behavioral Health Department. The mission of PROS is to provide person-centered, holistic, recovery-oriented care so individuals can achieve their life goals. This employee will have the opportunity to provide direct care and advocacy for a diverse and vulnerable population. The PROS team specializes in a wide range of treatment interventions, including trauma-informed care and integrated mental health and substance use treatment.
Responsibilities:
Provides direct care to a diverse population of people diagnosed with Serious Mental Illness (SMI).
Per diem caseworker role is designed to facilitate groups and assist with individual caseload coverage when team members are out of the office, including providing crisis intervention, de-escalation, and risk assessment to any participant who requires immediate assistance.
Provide group, supportive, and goal-directed counseling based on the treatment need of each individual participant; this includes providing vocational services, dual recovery services, creative and expressive arts services, evidence-based treatments such as CBT, DBT, ACT, and other clinically appropriate interventions through a person-center, strengths-based, and trauma-informed lens.
Group services include but are not limited to: Basic Living Skills Training, Benefits and Financial Management, Community Living Exploration, Crisis Intervention, Engagement, Individual Recovery Planning, Information and Education Regarding Self Help, Structured Skill Development and Support, Ongoing Rehabilitation and Support, Coping Skills Training, Disability Education, Dual Disorder Education, Problem-Solving Skills Training, Relapse Prevention Planning, Integrated Dual Disorder Treatment, Intensive Rehabilitation Goal Acquisition, Intensive Relapse Prevention, and Family Psychoeducation .
Meeting with individuals to assess safety by utilizing suicide risk assessment, CSSRS and violence risk assessment to assess suicide/homicide risk and safety to provide accurate interventions to mitigate harm to patient and community
Meeting with individuals to assess determinants of health and assist in making referrals for appropriate services, including but not limited to housing, transportation, medical providers, and care management
Adheres to all OMH, Joint Commission, Department of Health, Medicaid & Medicare regulations.
Perform other duties as assigned
What you will need:
A Bachelor's degree in Psychology, Social Work required.
Master's in Social Worker preferred.
Mental health experience preferred.
Pay Range: $23.40 - $33.69
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
