Schedule: M-F 8:30 am - 5 pm Weekend On Call Rotation and Holiday Rotation

Territory: Columbus, OH, Brice, Grove City, and surrounding areas

HOW YOU'LL WORK

  • Provide comprehensive skilled services as ordered by the attending physician

  • Educate patients and their family members while ensuring patient safety

  • Complete accurate documentation and remain compliant with all legal and regulatory requirements

QUALIFICATIONS & ATTRIBUTES

  • Current Physical Therapist license in the state of employment

  • Minimum of one year of experience as a PT in an acute setting (required)

  • Home care experience preferred

  • Strong communication and interpersonal skills

  • Knowledge of therapy best practices

  • Ability to make sound clinical judgments

  • Problem-solving skills and adaptability

  • Detail-oriented and observant

  • Strong work ethic with the ability to prioritize and be timely

  • Valid driver’s license and auto insurance (must be listed driver)

MAJOR AREAS OF RESPONSIBILITY

  • Provide skilled physical therapy in patients’ homes as ordered by the attending physician

  • Observe, record, and report patient reactions, changes, and progress

  • Perform OASIS assessments and adjust plans of care as needed

  • Complete initial evaluations and admissions within 48–72 hours of referral

  • Develop and manage individualized plans of care

  • Help decrease re-hospitalizations, especially for high-risk patients

  • Treat patients to relieve pain and restore function using physical methods (exercise, massage, heat, water, etc.)

  • Arrange outpatient services when care cannot be provided in-home

  • Promote continuity of care (admissions, transfers, discharges)

  • Educate and counsel patients and families on conditions and care plans

  • Involve patients and families in plan of care development

  • Instruct patients on proper use of assistive devices (wheelchairs, walkers, crutches, etc.)

  • Ensure patient/family understanding of financial responsibility for equipment

  • Supervise PTAs and, when appropriate, nurse assistants

  • Evaluate patient response and adjust plan of care accordingly

  • Communicate with PTAs and nurse aides regarding supervisory visits

  • Complete all documentation per agency and Medicare/Federal guidelines

  • Participate in audits, meetings, and training sessions

  • Maintain active licensure and professional standards

ESSENTIAL FUNCTIONS

  • Travel to patient locations as scheduled

  • Maintain daily attendance at assigned visit locations

  • Complete documentation within required timeframes:

    • Within 24 hours for routine visits

    • Within 48 hours for start-of-care visits

  • Submit all required documentation within company standards

  • Establish and follow plans of care within scope of practice

Health Requirements

  • Must meet all physical job requirements

  • Provide proof of a current negative TB test (or complete testing at orientation)

  • Annual TB testing required unless prior documentation exists

  • Must be free of conditions that would limit job performance

  • May be exposed to bloodborne pathogens
