Location: Must reside in Dallas TX or near the counties of Ellis, Collin, Hunt, Kaufman, Navarro, or Rockwall, TX.
NOTE: Work from home and IN-PERSON member visits are required - 3 days per week
Business Unit: STAR+PLUS Service Coordination, IDD Waiver Team
Position Purpose: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
Education/Experience: Requires a Bachelor's degree and 2 – 4 years of related experience or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.
Preferred Experience:
• 3+ years of case management experience working directly with the IDD members and IDD Waiver Providers
• 2+ years of experience coordinating and managing medical and social services, and providing patient advocacy and education to Medicaid members
• Experience in FIELD-BASED Social Worker or Case Managers role in community health mental health or state social services settings, such as, Local Intellectual Developmental Disabilities Authority (LIDDA), Mental Health Authority (MHA), or Behavioral Health Authority is preferred.
