This position is a collective bargaining position 

  • Day Shift 

  • 5 Days per week 

  • Full time 40 hours 

Schedule: 5x8 hour shifts. 

Position Summary

The Intake Nurse, under the supervision of the Clinical Nurse Supervisor-Intake, acts as a contact for information and arrangement of all services from physicians, HMOs, discharge planners and other outside referral sources. The Intake Nurse provides access for new patients to Hospice and Home Health services.


Position Specific Responsibilities

  • • Receives case referrals from physicians, HMO insurers, discharge planners, etc., ordering hospice services:

    • For home care referrals, takes hospice evaluation and admission order and assures appropriate staff are notified to follow up that all services and products are arranged.

    • Follows up with referrals to confirm that all services and products have been arranged.

    • For home care referrals takes orders and ensures appropriate people are notified and follows up with referrals to confirm that all services and products have been arranged.

    • Assesses patient appropriateness for admission based on intake data and admission criteria.

    • Provides information on services to physicians, HMOs, insurance companies, discharge planners and other referral sources.

    • Assesses customer service needs and ensures linkage with appropriate department and program.

    • Maintains productivity standards as set by organization guidelines.

    • Documentation / Information Management:

    • Assigns medical record number and logs referral according to office protocol.

    • Enters initial referral information into computer system for patient tracking.

    • Adheres to current policies and contractual arrangements.

    • Adheres to record keeping system for managing telephone intake services.

    • Collects data on callers’ requests for information and services.

    • Leadership / Supervision:

    • Participates in orientation, education and development of team members as directed by supervisor.

    • Development:

    • Participates in appropriate continuing education as may be requested and/or required by immediate supervisor.

    • Accepts personal responsibility for other educational activities to enhance job-related skills and abilities.

    • Performance Improvement:

    • Receives grievances, problems, and complaints from callers and follows up with appropriate departments to ensure satisfactory resolution.

    • Adds to log of complaints, noting department and nature of complaint.

    • Monitors current and potential referral sources for feedback on performance.

    • Participates in the Performance Improvement Program and participates in and adheres to the Corporate Integrity Plan.

    • Budgetary:

    • Follows Core Value for being financially prudent.

    • Performs other duties as assigned.


Education and/or Experience

  • Required - Coursework/Training Degree/diploma from accredited school of nursing.
  • Preferred - Bachelor's Degree Nursing
  • Required - 1 year Related work experience in hospice, palliative care, home health, acute care &/or skilled nursing.


Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Demonstrates effective and diplomatic telephone communication skills.
  • Computer Skills: Demonstrates a working knowledge of computer applications including Microsoft Office and e-mail.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Other Skills and Abilities: Demonstrates knowledge of home care products and services.


Certifications, Licenses, and Registrations

  • Preferred upon hire: Provider BLS - American Heart Association
  • Required upon hire: California Registered Nurse License (Vendor Managed)

  • Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $55.30 - $89.27

  

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
