Position Summary

The Lead Intake Coordinator oversees and supports the daily operations of the Intake team to ensure the smooth and timely processing of referrals within home health and/or hospice programs. This role coordinates staffing, manages referral workflow, verifies insurance coverage, and ensures accurate documentation in the electronic medical record (EMR). The Lead Intake Coordinator collaborates with clinical and billing teams to support timely patient admissions, provides training and guidance to intake staff, and promotes positive working relationships with internal teams, patients, families, and external referral sources.


Position Specific Responsibilities

  • • Coordinate, oversee and guide the daily activities of the Intake team.

    • Ensure adequate staffing to meet internal and external customer needs and maintain excellent customer service.

    • Prioritize daily referrals among team members to maximize capacity and ensure timely patient admissions.

    • Accurately transcribe medical and insurance information from referral documentation and ensure pertinent documents are scanned into the EMR.

    • Verify and obtain physician orders, including validation of new physician NPIs.

    • Evaluate insurance coverage to ensure appropriate financial reimbursement.

    • Communicate with clinical teams and the billing department to coordinate services.

    • Oversee the Medicare insurance verification process and ensure timely completion of insurance reverification.

    • Assist with the orientation and training of new personnel in the intake department.

    • Provide performance feedback on intake staff to the department manager.

    • Complete required compliance education.

    • Perform other duties as assigned.

Education and/or Experience

  • No Requirement
  • Required - 2 years Home Health or Hospice intake experience.
  • Preferred - Prior home health management position.


Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy. Excellent customer service, interpersonal and communications skills. Good organizational skills. Knowledge of home health compliance issues, payer coverage requirements related to eligibility for services. Knowledge of home health/hospice referral process preferred.


Certifications, Licenses, and Registrations

    • Driving may be necessary as part of this role.
    • Caregivers are required to comply with all state laws and requirements for driving.
    • Caregivers will be expected to provide proof of driver license and auto insurance upon request.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Compensation range: $31.27 - $47.86 

  

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
