Overview
The Clinical Pharmacist will be under the supervision of the Department Director or his designee. Must successfully complete a Departmental Pharmacy Exam, a certification in Basic Life Support & Advanced Cardiac Life Support may be required, or an ASHP accredited Residency in Pharmacy Practice &/or a Residency in a Pharmacy Practice Specialty preferred or Masters of Science Degree in Pharmacotherapy.
Responsibilities
- Monitor patients to maximize drug efficacy & minimize drug toxicity.
- Provide safe preparation and distribution of drugs in accordance with all regulatory agencies.
- Verifying and reviewing medical orders
- Detect & report medical errors and Adverse Drug Reactions.
- Serves as chair, director or team leader in committees, special projects, or clinical programs.
- Directs and leads Pharmacists, Pharmacy Technicians, and Pharmacy Interns.
- Design and collect data for Medication Use Evaluations and Quality Assurance Surveys.
- Provides pharmacokinetic, pharmacotherapy, or pharmacy-related consultations. Disseminates drug information by teaching and lecturing health care professionals and patients.
- Participate in emergency codes, dept's various consultative services, research endeavors, multidisciplinary medical attending rounds and medication-related hospital committees.
- Administering Medical histories, Drug Discharge Interviews and lectures to healthcare professionals & patients.
- All other duties as requested.
Qualifications
Education:
- Doctor of Pharmacy Degree from an ACPE accredited college of pharmacy and completion of an ASHP accredited PGY-1 Pharmacy Residency Program required.
Experience:
- Minimum three (3) years of satisfactory experience in the practice of pharmacy.
Knowledge and Skills:
- Good interpersonal skills.
- Ability to read, write and interpret documents.
- Ability to communicate effectively with patients and/or co-workers.
- Knowledge of all codes and demonstrate understanding.
Licenses, Certifications:
- Licensure as required by New York State guidelines.
Physical Requirements:
- Position requires prolonged periods of standing, reaching, walking throughout the working day.
- Position will be required to stoop, bends, lifts, carry items weighing up to 25 pounds for routine performance of essential functions.
- Able to lift up to 100 pounds, if necessary, to assist lifting and moving patients.
- An individual in this position may be exposed to: Adverse indoor and outdoor environmental conditions, Blood borne pathogens, Physical hazards (e.g., needle pricks, etc.), and Chemical hazards.
