Overview

The Case Management Assistant provides hands-on post-acute care planning services to hospitalized and emergency department patients, as assigned by an RN Case Manager or Social Work Case Manager, in accordance with department policies and procedures

Responsibilities

  • Support RN and Social Work Case Managers in post-acute care arrangements, ensuring compliance with regulations.
  • Coordinate the transfer of complete medical records for each post-acute destination during business hours.
  • Communicate with post-acute care facilities about bed availability via CarePort. Communication may also include fax or phone
  • Provide regulatory notices (e.g., Important Message from Medicare, Choice Forms) to patients and families, and file documents as required.
  • Notify RN Case Manager or Social Work Case Manager of the patient’s post-discharge care choices promptly.
  • Arrange transportation, as required, and communicate transfer details to patients and families.
  • Arrange for durable medical equipment in the patient’s home.
  • Schedule post-discharge physician appointments.
  • Document accurate data, authorizations, referrals, entries into CarePort and/or medical record in a timely manner.
  • Act as liaison between RN and Social Work Case Managers to ensure information flow.
  • Establish collaborative relationships with case management staff, vendors, liaisons, patients, families, and caregivers.
  • Conduct post-discharge follow-up calls and refer patient concerns to the appropriate Case Manager

    Qualifications

    Required:

    • High school education or equivalent required

    Preferred:

    • CarePort and Meditech system skills preferred

    Other Skills:

    • Excellent clerical and organizational skills
    • Excellent communication/people skills in person and on telephone
    • Familiar and comfortable with medical terminology
    • Computer and data entry skills; proficient in Word

    Must be able to read, write legibly, speak, and comprehend English.

    Work Context

    • Frequent telephone contact with providers, provider office staff, hospital staff, other customers, and post-acute provider partners
    • Daily and close contact with management and care management staff and community agencies.

    Physical Factors

    • Requirements: Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
    • Must be able to perform the duties with or without reasonable accommodation.
    • Hearing and vision must be normal or corrected to within normal range.
    • Physical presence onsite is essential.
