Job Summary and Responsibilities

Are you an LPN passionate about delivering exceptional patient care in a collaborative, multi-disciplinary environment? We're seeking a skilled and empathetic LPN to join our team, providing hands-on care and directly impacting patient well-being.

 

As an LPN, you will:

  • Provide Direct Patient Care: Observe, collect, and record patient data to inform comprehensive care plans.
  • Collaborate: Work closely with RNs, physicians, patients, and families to implement age-appropriate care.
  • Educate: Empower patients and families with tailored information on medications, procedures, and discharge needs.
  • Administer Medications: Safely administer medications based on your skills and competency.
  • Ensure Safety: Implement fall prevention strategies and verify equipment readiness.
  • Document & Monitor: Track patient progress and maintain accurate records.
  • Support & Learn: Share knowledge with colleagues and participate in continuous education.
  • Adapt: Be flexible, willing to float to other areas, and take on assigned duties.

Job Requirements

What We Require:

  • Education: Completion of an accredited, state-approved LPN program
  • Licensure: Active LPN license in Oregon
  • Certifications: AHA BLS for Healthcare Providers (Required within 30 days of hire)
  • Skills: Ability to work in a dynamic environment, adapt to interruptions, and possess strong communication and interpersonal skills.
