Job Summary: The Support Services Coordinator is responsible for performing various administrative tasks related to the care of individuals served. This includes monitoring contracts and budgets, evaluating family satisfaction with services provided and completing required documentation for assigned caseload.

Responsibilities

Dependable

  • Effectively responding to and managing crisis situations, which may include client physical aggression, utilizing prescribed verbal intervention or physical management techniques.
  • Monitoring independent contractor hours to ensure service match the family’s needs and do not exceed the NOA.
  • Reviewing contractor invoices and processing for payment.
  • Evaluating contracted services to ensure independent contractor is fulfilling contract terms and complying with regulatory guidelines.

Active

  • Assisting families with selecting an independent contractor to provide services as defined by the NOA.
  • Ensuring ISP/BSPs of assigned clients are implemented.
  • Conducting bi-weekly assessments to ensure client and family satisfaction.
  • Serves as an active member of an interdisciplinary treatment team, completing contract evaluations and addressing client concerns.
  • Attends mandatory staff meeting and other meetings as assigned.
  • Performing other duties as assigned.

  • Showing initiative by identifying solutions to work towards improving stakeholder satisfaction.
  • Prioritizing duties in a manner consistent with organizational objectives and growth.

Accountable

  • Notifying appropriate management and regulatory agencies of incidents within the required timeframes as determined by regulatory agencies and/or Damar policies.
  • Monitoring client’s finances and ensuring expenses are paid in a timely manner, if required.
  • Auditing client files on a monthly basis or more frequently as needed.
  • Monitoring the services provided to assigned clients by ensuring contractors meet regulatory requirements and client support plans approved by client and/or family.
  • Serving as a representative of clients in Court Reviews, Quarterlies, Treatment Team meetings and Case Conferences by providing accurate information regarding the client’s treatment progress and areas of need.
  • Completing monthly reports and any other administrative documentation required.
  • Ensures updated and thorough documentation is completed for each assigned independent contractor and client.  Documentation is expected to be accurate and legible.
  • Completes required tasks to ensure assigned department meets quality improvement, evaluation and reporting requirements.
  • Will adhere to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual.
  • Completes and updates mandatory training and other licensure related items in a timely manner.
  • Maintains documentation that is accurate and legible.
  • Completing incident reports, including reporting any suspected exploitation, abuse, or neglect of a client.

Qualifications

Education & Experience: High School Diploma with four years of experience in a human services field OR Two years of college combined with two years of work in a human services field OR Bachelor’s degree.

 

Equipment Familiarity: Proficiency with MS Office applications including: Word, Excel, Outlook and PowerPoint.

 

Certifications, Licenses or Special Training:

  • Minimum age of 18 depending on program and / or department requirements.
  • Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Checks.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier.
  • Must have or be able to obtain First Aid/CPR certification.
  • Must have or be able to obtain Critical Moments Training (CMT) certification.
  • Successful completion of pre-employment drug screen and physical required.
