Job Summary and Responsibilities

The Public Health Nurse (PHN) is responsible for the delivery of patient care and delivery of services through grant programming within the scope of professional nursing standard.  The PHN uses the nursing process (assessment, planning, intervention, and evaluation as a systemic approach to the direct and indirect care to clients and families, which promotes quality, continuity and safety for the client, family and community.  The PHN practices with the scope of the Minnesota Nurse Practice Act and within Minnesota Department of Health guidelines, Local Public Health Act and departmental policies, procedures and guidelines.  The PHN works under the leadership of the PHN Community Health Supervisor.  

 

Essential Key Job Responsibilities

  • Deliver public health and case management services to clients using initiative, judgment, and independent decision making within the scope of professional nursing standards.
  • Admission and visits for Rural Health Clinic Nurse program 
  • School Nurse assignments as directed by PHN Community Health Manager. 
  • Complete grant work plan activities based on the grant requirements as assigned 
  • Utilize policies and procedures when performing assigned activities
  • Chart according to grant and community health guidelines
  • Direct and guide the activities of other health personnel 
  • Attend required trainings/meetings
  • Link the client with appropriate providers and resources throughout the continuum of health and human services and care settings.
  • Provide culturally sensitive, evidence-based health education to clients and families
  • Job Requirements

    • Graduate of an accredited school of professional nursing with current licensure in the State of Minnesota.
    • 1 – 3 years public health experience and/or home care
    • MN RN LICENSE
    • BLS
    • Public Health Certification
    • Excellent communication and customer service skills, basic computer skills
