Position Overview

An established outpatient rehabilitation clinic in Peoria, AZ is seeking a full-time Physical Therapist to join its innovative and collaborative team. This role features a hybrid practice model, blending in-person care with telehealth visits to improve patient access and enhance clinical flexibility.

Compensation & Benefits

  • Base Salary: $85,000 – $100,000 annually (commensurate with experience)
  • Monthly Performance Bonus: $500 – $2,000 (RVU-based)
  • Sign-On / Relocation Bonus: $50,000 for qualified candidates

Key Responsibilities

  • Conduct thorough patient evaluations both onsite and through telehealth platforms
  • Develop and implement individualized treatment plans using manual therapy, therapeutic exercise, and digital tools
  • Track patient outcomes and modify treatment plans as needed
  • Provide patient education on home programs, injury prevention, and recovery strategies
  • Complete accurate, timely documentation using EMR
  • Collaborate with PTAs, physicians, and other interdisciplinary team members
  • Participate in clinic initiatives, quality improvement efforts, and continuing education

Candidate Requirements

  • Graduate of a CAPTE-accredited U.S. Physical Therapy program (Puerto Rico programs not eligible)
  • Master’s degree required; Doctor of Physical Therapy (DPT) preferred
  • Active or license-eligible as a Physical Therapist in Arizona
  • Strong interpersonal, communication, and clinical reasoning skills
  • Comfortable delivering care in both in-person and telehealth settings (training provided)
  • Committed to high-quality, patient-centered, evidence-based care
