Responsibilities:

• Conduct patient visits in the home care setting
• Timely Documentation of patient visits in EMR
• Perform routine, respite, inpatient, admission, and recertification visits
• Attend IDT meetings, and coordinate plan of care with interdisciplinary team
• Assists in development, review and revision of the plan of care.
• Provides nursing care in accordance with physician’s plan of treatment, and individual plan of care.
• Observes, records and reports any changes in patient's condition to appropriate personnel and/or physician.
• Provides ongoing instruction and supervision of patient, family or other health team personnel in the plan of treatment.
• Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required.

Requirements

• RN License in the state of TX
• 1-2 year of RN experience in a clinical setting preferred
• 1 year hospice experience preferred
• Active Driver's License
