Saturday & Sunday, 7 am - 7 pm, or 7 pm to 7 am
Primary Job Responsibilities
- Providing assistance, comfort, and companionship to frail, ill, or immobile clients.
- Executing the patient care plan and maintaining communication with the RN Case Manager.
- Monitoring and reporting changes in the physical, mental, and emotional conditions of clients to designated parties.
- Performing personal hygiene care and light housekeeping duties within the patient's living area.
- Following nutritional guidelines and assisting clients with meal preparation and feeding.
- Assisting with incontinence products and providing bathroom support.
- Running errands for clients, including grocery shopping.
- Providing mobility assistance for clients.
- Accompanying clients on trips with prior approval from the Staffing Coordinator.
- Maintaining a clean, safe, and orderly environment for clients.
- Documenting care using the Agency-approved Electronic Visit Verification system.
- Communicating any scheduling needs to the Staffing Coordinator.
- Administering medications and treatments as prescribed, following proper authorization and training.
- Ability to read, write, and communicate effectively
- Expertise in trach and vent management
- Capability to understand written and verbal instructions
- Graduate of an accredited LPN program
- Current LPN license (PA)
- Successful completion of a job-specific knowledge exam
- Strong organizational skills
- Compassionate attitude towards patient care
- Detail-oriented documentation abilities
- One year of home health experience preferred
- Familiarity with feeding tubes, assisted breathing devices (ventilators), and Hoyer lifts
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
- Valid driver's license and auto insurance required for employees in driver status.
