Overview

Provides assessment, support, information and referral, advocacy services, and other resource assistance to patients and their caregivers, directly or through consultation, within a multi-disciplinary outpatient team.

Responsibilities

  • Provides support to pediatric patients and families, in an outpatient clinic setting as part of a multidisciplinary team, to help address psychosocial needs within the context of the patient’s medical care.
  • Completes psychosocial assessments of pediatric patients and families to assess current supports, strengths, areas of need, and barriers to accessing care.
  • Coordinates and connects patients and their caregivers with community resources to address/meet identified needs. Serves as a link between the medical team and the patient’s community/natural supports.
  • Assists caregivers in adapting to difficult and complicated dispositions involving complex care needs, multiple agencies/providers and linkage to resources in the community.
  • Provides consultation, assessment, referral and coordination in cases of suspected child abuse or neglect.
  • Organizes support groups, provides psychoeducational materials and opportunities for patients and their caregivers, based on their individual medical needs.
  • Documents objective, concise and pertinent clinical information in the medical record.
  • Perform other duties as assigned.
  • Qualifications

    • Master’s Degree in Social Work (MSW) required.
    • Social work experience in medical, school, or developmental services setting preferred.
    • Strong organizational skills, verbal and written communications skills.
    • Sound decision making and judgment.
    • Time management skills.
    • Negotiation skills.
    • Proven ability to deal with conflict resolution.
    • Comfortable with basic computer skills.
