Summary:

Perform advanced level functions in the Health Access Nurturing Development Services (HANDS) program providing Parent Visit Assessment/Survey with parents (prenatal and/or immediately after the birth) to determine appropriate level of support from the community. This position will conduct family home visits, assess needs, provide education, connect families to resources, and support our mission of improving community health outcomes.

Description:

Key Responsibilities

  • Provide services under the HANDS program, Harm Reduction, and other assigned initiatives.
  • Contact referred families within 7 days and conduct assessment visits within 30 days of referral.
  • Complete home visits weekly or biweekly, providing guidance, education, and support to families with infants and toddlers (birth to age 3).
  • Document case records, visit logs, and required forms accurately and in a timely manner.
  • Participate in staff meetings, trainings, strategic planning, and quality improvement initiatives.
  • Facilitate family connections to community resources and services.
  • Conduct quarterly visits to provide additional professional support to Family Support Workers.
  • Maintain confidentiality in compliance with HIPAA and follow all state and federal regulations.
  • Provide basic nursing care, health education, and counseling consistent with public health standards.
  • Attend trainings, conferences, and family events as required (travel expenses reimbursed per state policy).

Qualifications

  • Current Registered Nurse (RN) license in Kentucky (copy required on file) or Certified Social Worker.
  • Bachelor’s degree in nursing or related field preferred.
  • Valid driver’s license, reliable transportation, and automobile insurance required.
  • Ability to work independently while collaborating with supervisors and team members.
  • Strong organizational and communication skills.
  • Commitment to maintaining confidentiality and providing culturally sensitive care.
  • Must pass a criminal background check.
