ABOUT THIS JOB

Position Specific Summary

The Care Coordinator position at the City of Tucson's Department of Housing and Community Development is responsible for providing in-home and community-based case management to Choice Neighborhood Implementation participants and/or families residing in shelter and supportive housing programs. This position will ensure participants are connected to the services, provide stabilization through resources, and opportunities they need to thrive and meet programmatic goals: improved participant health and wellness, economic stability, education, and housing stability. 

Work is performed under the supervision of the Community Services Project Supervisor. This position does not supervise.

Duties and Responsibilities

  • Maintains an active caseload and delivers in-home and community-based case management to residents by: conducting annual or as needed assessments to identify needs and strengths, developing and implementing individually tailored care plans, facilitating and monitoring referrals to community resources to ensure residents’ needs are met and follow-up is completed as necessary, and collaborating with community partners to promote housing stability.

  • Coordinates , promotes and recruits resident for participation in on-site and community programs, services, and activities.

  • Partners with redevelopment and relocation teams to educate residents on their rights, responsibilities, and opportunities during relocation, supporting successful transitions and maximizing re-occupancy of redeveloped units.

  • Maintains participant files, records and documentation in accordance with City policies and federal requirements including privacy and confidentiality standards (e.g., HIPAA, if relevant). Collects and compiles data for grant applications, compliance reports, and local, state, and federal reporting obligations.

  • Participates in collaboratives and professional development opportunities relevant to assigned service areas. Shares and researches best practices and community resources related to diverse resident populations, including families with children, older adults, and individuals with disabilities.

  • Transports residents in city owned vehicles to appointments, events, and other locations as needed.

  • Performs all other duties and tasks as assigned.

Work conditions as follows:

  • Mostly office environment.

  • Availability for occasional weeknight or weekend engagements as needed.

  • Able to travel to participant’s homes or public locations for case management visits.

  • Able to conduct regular home visits, which may include entering environments requiring the use of personal protective equipment (PPE).

MINIMUM QUALIFICATIONS

MINIMUM REQUIRED QUALIFICAITONS

Education: Bachelor’s degree in related field
Experience: Two (2) years of relevant experience.
 
*Any combination of relevant education and experience may be substituted on a year-for-year basis.
 
Valid and Unrestricted Class D Drivers License


PREFERRED QUALIFICATIONS

Degree in Social Work, Human Services, Public Health, Psychology, Sociology, or a closely related field.

Experience in case management, crisis deescalation, care coordination, or social service delivery, preferably in affordable housing, community-based settings or behavioral health settings.

Experience working with diverse populations, including individuals with disabilities, severe mental illness, substance use disorders, older adults, and families with complex needs using a trauma-informed approach.
