• Reports to physical therapist when re-evaluations are needed and or changes in patient status.
• Delegates tasks and duties as indicated.
• Performs documentation per policy and procedures ( progress notes, discharge summary, and multi-disciplinary education and treatment plan forms).

Required:

• PTA Licensure in practicing State
• Associates as Physical Therapy Assistant

Preferred:

• Physical Therapy Assistant or Rehab experience
