Job Description:
Job Summary
The Dementia Care Navigator supports individuals and caregivers who suspect the presence of dementia or Alzheimer’s. This role involves administering a non-clinical screening tool, coordinating long-term support services, making appropriate referrals, and connecting clients to community resources.
The position reports to the Office on Aging Client Services Manager.
Essential Job Functions
Administers non-clinical screening tool in person or over the telephone with individuals where dementia or Alzheimer’s is suspected.
Provides information and assistance to older adults and the general public on long-term support and dementia-related resources.
Provides information and assistance to appropriate clients through the Maryland Access Point (MAP) program.
Interviews individuals in person or by telephone to assess their medical, social, and financial needs to determine eligibility.
Completes Level I screenings as appropriate.
Develops and promotes outreach events; promotes various programs.
Offers support and resources to caregivers and older adults.
Coordinates referrals to external agencies to implement services for individuals.
Maintains a case record for each participating individual; records and maintains records.
Prepares monthly and statistical demographic reports.
Completes forms for individuals as needed.
Attend meetings related to the position.
Attends forums, seminars, and trainings.
Maintains a resource file pertinent to the position.
Promotes programs throughout the county to include underserved areas.
Other Duties
Collaborates with internal staff and external community partners and agencies.
Performs related work as required.

Required Qualifications (Note: Any acceptable combination of education, training and relevant experience that provides the above knowledge, abilities and skills may be substituted on a full-time year for year basis.)
Training and/or Education:
Associate’s Degree in a social or behavioral science, or healthcare field.
Experience:
Two years of experience in healthcare, social services, or related fields.
Licenses or Certificates:
Valid driver’s license.
Special Requirements:
Subject to a background investigation.
Operation of a county-owned vehicle.
Physical Demands:
May require periods of standing, walking, repeated bending, crouching, stooping, stretching, or reaching; lifting of objects up to 49 lbs.; operation of keyboard devices.
Unusual Demands:
Work is subject to frequent interruptions.
FLSA Status:
Nonexempt
Compensation Steps:
Pay rate: $26.25 per hour
Grade H18, Hourly position, up to 35 hours per week
