TItle: Clinical Documentation Specialist/Quality Assurance LPN

Location: Melville, NY

Salary: $75,000 / yearly
 

Overview:

We seek an experienced Licensed Practical Nurse (LPN) to join our Melville team as a Clinical Documentation Specialist. This individual utilizes clinical expertise to review medical records against appropriate criteria in conjunction with contract requirements, critical thinking, and decision-making skills to determine medical appropriateness while maintaining production goals and QA standards. Ensures day-to-day processes are conducted in accordance with LHCSA regulatory standards.
 

***The position is based full-time (Monday-Friday) on-site at our Melville headquarters.

Responsibilities:

• Familiar with DOH/home care regulations and requirements for standards of care related to documentation
• Responsible for day to day review of clinical nursing notes/documentation and ensuring documentation is placed into correct EHR files for billing and other purposes
• Participates in continuous quality assurance improvement exercises
• Assists intake and schedulers home care staff as needed to ensure patient care needs are met
• Validates completeness and accuracy of patients' eligibility and medical documentation
• Assist patients with questions and/or concerns regarding benefit coverage and appointment details, as needed
• Understand that healthcare delivery is continually evolving, and departmental needs and goals are subject to change; responsibilities may be expanded or restricted at the discretion of our Chief Regulatory Officer and/or Director of Administrative Compliance
• Other quality assurance duties may be assigned as needed
   

Qualifications:

• Must have current LPN licensure and maintain eligibility
• Previous experience in home care setting strongly desired
• Proficient in Microsoft applications and utilizing EHR systems
• Demonstrated experience in a QA/compliance role within a clinical setting
• Detail-oriented and organized
• Strong verbal and written communication skills
