Day Shift - 7.5 Hours (United States of America)
The HIV Patient Navigator provides patient-centered education, advocacy, and care coordination services to individuals living with HIV. This role supports patients in navigating the healthcare system, addressing barriers to care plan adherence, and facilitating access to clinical and community-based services. Working collaboratively with the healthcare team under the leadership of a Nurse Practitioner, the HIV Patient Navigator plays a key role in improving health outcomes and supporting continuity of care.
Essential Duties and Responsibilities
Education
- High School Diploma or equivalent required.
Experience
- Prior experience working with patients.
- Experience in a hospital or community health setting preferred.
- Experience working with the HIV-positive population or similar patient populations preferred.
Skills & Abilities
- Computer literacy required, including use of electronic health records.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Strong interpersonal, communication, and organizational skills.
- Ability to take initiative, start projects, and work effectively under the leadership of a Nurse Practitioner.
Salary Range: $27.00/hr - $35.00/hr
Wednesday, Thursday & Friday
8:00 AM – 4:00 PM
