Job Description Summary: 
The Healthcare Home (HCH) Licensed Practical Nurse (LPN) Care Coordinator is responsible for delivering supportive services to individuals with a variety of medical needs and behavioral health issues (including both substance use and mental health). They function as a supportive role to Nurse Care Managers and provide services to Healthcare Home clients to promote their dignity, independence, and overall well-being. The HCH LPN demonstrates a warm, welcoming, empathic, and hopeful attitude, conveying a philosophy of dual recovery.  They will be trained in trauma-informed care and will provide services in a sensitive and responsive environment to prevent victimization, abuse, or trauma to clients. They will work under the supervision of the Healthcare Home Director.

Education:
•    High School Diploma or equivalent 
•    Diploma in Practical Nursing
•    Current LPN license by the State of Missouri.

Responsibilities:
•    Meets with clients via phone, telehealth, or in person for various reasons, including but not limited to, enrollment and/or discharge, post hospitalization follow up, education, coordination of intake/annual screening and metabolic screening, confirmation of benefits, and eligibility determination.
•    Performs data control and data entry, develops/builds/updates files, handles special projects, files records/documentation, communicates with other disciplines as appropriate, and completes other duties as assigned.
•    Supports each Nurse Care Manager with client-related tasks for case management team.
•    Collaborates with the Healthcare Home Director to ensure that each team receives an equitable balance of support and to assure a fair balance of workload for all team members.
•    Completes all required documentation within established timeframes and according to quality standards set by State and Federal regulations, CARF certifications, and program expectations.  
•    Perform all other duties as assigned.

Physical Requirements:
•    Typical office environment with physical demands including prolonged sitting, frequent standing, bending, stooping, and occasional lifting of up to 50 pounds
•     Requires a normal range of hearing and manual dexterity sufficient to operate a keyboard, telephone, photocopier, calculator, lab equipment, and other office equipment as needed. 
•    Requires ability to work under stressful conditions, to work varied hours, and to deal with anxious or irate individuals.
•    TB Test upon hire.
