About Us – Social Services

• The Social Services team provides discharge planning to every patient who needs assistance with placements, equipment, services, social issues, etc

• Each inpatient department is assigned a Social Worker. This includes Freeman West, East and Neosho

• Our team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possible

What You’ll Do

Assists the Director of Social Services in the daily management of the Social Services Department. This will include West, East, and Neosho campuses. The Social Services Coordinator will also complete insurance authorizations for aftercare for skilled nursing facilities, acute rehab, and long-term care hospitals for all campuses.

Requirements

• LBSW or MSW, we will accept a four-year degree related to Social Work in lieu of a BSW or MSW.

Preferred Requirements

• Current Missouri License for LMSW.
