Sierra Valley Rehab Center is a premier skilled nursing facility in Porterville, CA, committed to delivering exceptional care to our residents. We are currently hiring compassionate and reliable Registered Nurses (RNs) to join our clinical team.

Position Summary:

As an RN at Sierra Valley Rehab Center, you will play a key role in supervising staff, coordinating resident care, and ensuring clinical excellence in accordance with federal, state, and facility regulations. This position involves both leadership and hands-on nursing responsibilities to support our residents’ health, dignity, and well-being.

Key Responsibilities:

• Supervise CNAs and LVNs during your shift

• Provide direct nursing care, administer medications, and perform treatments

• Develop and update individualized care plans based on patient needs and progress

• Document observations, interventions, and resident responses accurately and in a timely manner

• Participate in assessments, admissions, discharges, and care conferences

• Respond to changes in residents’ conditions and coordinate care accordingly

• Ensure compliance with all facility policies, procedures, and state regulations

• Promote a culture of compassion, professionalism, and teamwork

Qualifications:

• Active and unencumbered RN license in the state of California

• Graduate of an accredited nursing program

• Current CPR certification

• One year of experience in a skilled nursing or long-term care facility preferred

• Strong assessment, clinical, and communication skills

• Proficient in electronic documentation systems (PCC experience a plus)

Physical Requirements:

• Ability to lift 25+ lbs and assist with patient mobility

• Must be able to stand and walk for extended periods

• Ability to respond quickly and effectively in emergency situations
