Physical Therapist Assistant (PTA) – Sonoma Post Acute

Location: Sonoma, CA
Pay: $45 to $50 per hour
Schedule: Full Time, Part Time, or Per Diem

Sonoma Post Acute is seeking motivated and compassionate Physical Therapist Assistants (PTAs) to join our rehab team. We are looking for professionals who are passionate about patient care and ready to support residents in reaching their highest level of independence.

Responsibilities

• Provide therapy services under the supervision of a licensed Physical Therapist

• Assist in implementing individualized treatment plans for residents

• Support patients with mobility, strength, and functional activities

• Document progress and maintain compliance with regulations

• Work closely with the rehab team to promote recovery and independence

Qualifications

• Current California PTA license

• Strong communication and teamwork skills

• Passion for patient-centered care

• Skilled nursing or post-acute care experience preferred (new grads welcome)
