Registered Dietitian – Sacramento Post Acute

Location: Sacramento, CA
Pay: $88,600 – $96,500 per year, DOE
Schedule: Full-Time | Monday – Friday, 40 hours per week, flexible hours

Sacramento Post Acute is a 5-Star, 94-bed skilled nursing and subacute facility recognized for excellence in trach care, ventilator management, wound care, short-term rehabilitation, and long-term care. Our Dietary Department earned the County Food Safety Award of Excellence and is supported by a stable, experienced team. We are seeking a compassionate and motivated Registered Dietitian to join our leadership team.

Responsibilities

  • Conduct nutritional assessments and develop individualized dietary plans

  • Collaborate with the Dietary Manager to meet residents’ nutritional needs and goals

  • Adjust menus using menu services as necessary

  • Participate in weekly IDT meetings focusing on weight and skin integrity

  • Review patient charts and provide medical nutrition therapy recommendations

  • Assist with care planning, discharge planning, and quality assurance initiatives

  • Ensure compliance with state, federal, and facility regulations

Qualifications

  • Active California license as a Registered Dietitian required

  • Minimum 1 year of experience in a skilled nursing facility, hospital, or related healthcare setting

  • Strong knowledge of clinical nutrition and regulatory standards

  • Excellent communication, organization, and teamwork skills

  • Commitment to confidentiality, compassion, and resident-centered care
