St. Francis Healthcare is seeking a compassionate and dedicated Social Services Assistant to support our social services programs. In this role, you will assist the Social Service Director in enhancing the quality of life for our residents by addressing their medically-related social and emotional needs.
Responsibilities:
- Program Support: Assist the Social Services Director in planning, developing, organizing, implementing, and evaluating social service programs within the facility.
- Resident Care Planning: Participate in the development and review of individual care plans, working with residents, families, and the interdisciplinary team to identify and address psychosocial, mental, and emotional needs.
- Advocacy and Support: Provide emotional support to residents and families during their stay and assist residents in achieving the highest practicable level of independence and well-being.
- Communication and Coordination: Facilitate effective communication between residents, families, and staff. Serve as a liaison with community agencies to ensure residents have access to necessary services.
- Discharge Planning: Assist with coordinating discharge plans to ensure a smooth transition for residents returning home or moving to other care settings.
- Resource Management: Help residents and families understand and access financial assistance programs such as Medicare and Medicaid.
- Administrative Duties: Perform administrative tasks, including completing necessary forms, reports, and documentation related to the social services program.
- Policy and Compliance: Assist in the development and review of departmental policies and procedures to ensure compliance with all applicable regulations.
Qualifications:
- A high school diploma or equivalent is required.
- Prior experience in a healthcare or skilled nursing facility is preferred.
- Experience in a social services setting is a plus.
- A compassionate, patient, and empathetic demeanor with a strong desire to help others.
- Excellent communication, interpersonal, and organizational skills.
- Proficiency with basic computer applications and electronic health records (EHR) systems.
- Ability to work effectively both independently and as part of an interdisciplinary team.
