Oversees weekly PDPM meetings for skilled patients and monitors facility use of PDPM tools such as Physician Diagnosis Verification, CSR(GG), and Point Right.

Tracks and analyzes Quality Measures (QM) and QASP performance monthly and quarterly, providing clinical recommendations for optimal outcomes.

Provides comprehensive PDPM and MDS training to new MDS Coordinators to ensure regulatory compliance and data accuracy.

Ensures accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change evaluations.

Coordinates interdisciplinary participation in the completion of MDS and resident care conferences within required regulatory time frames.

Reviews and evaluates care plans for completeness, accuracy, and effectiveness, ensuring individualized resident goals and interventions.

Manages all level-of-care changes, notifies departments, and transmits required documentation to appropriate agencies.

Maintains data integrity through accurate entry, electronic submission, and correction of MDS assessments as needed.

Protects resident confidentiality and ensures all documentation meets federal, state, and medical standards.

Supervises assigned staff and ensures all licensing, continuing education, and professional standards are maintained in compliance with state nursing regulations.
