Your Impact

As an LVN at Sunnyvale Gardens Post Acute, you will play a vital role in supporting residents’ recovery and overall well‑being. You’ll work alongside a collaborative, caring clinical team in a supportive environment where your contributions truly matter.

Key Responsibilities

• Provide high‑quality nursing care to residents under RN and provider supervision

• Administer medications and treatments in accordance with care plans

• Monitor and document resident health status, reporting changes promptly

• Assist with assessments, care planning, and resident education

• Ensure compliance with all facility, state, and federal guidelines

• Promote a positive, respectful, and compassionate care environment

Qualifications

• Active California LVN license

• Current CPR/BLS certification

• Strong clinical and communication skills

• Ability to multitask and maintain accuracy in a fast‑paced environment

• Previous experience in post‑acute, skilled nursing, or long‑term care preferred but not required
