Gateway Post Acute – Porterville, CA
Salary Range: $70,000 – $72,000 annually (DOE)**
Gateway Post Acute is seeking a highly organized and clinically strong MDS Nurse (LVN) to oversee and coordinate the MDS process in our skilled nursing facility. This position plays a critical role in regulatory compliance, accurate reimbursement, and ensuring resident-centered care planning.
The ideal candidate understands the financial and clinical impact of precise documentation and thrives in a collaborative, fast-paced post-acute environment.
Coordinate and complete MDS 3.0 assessments and Care Area Assessments (CAAs) in accordance with federal and state guidelines
Ensure accurate, timely documentation to support Medicare and Medicaid reimbursement
Review clinical documentation to validate coding accuracy and optimize case mix
Collaborate with nursing, therapy, social services, and dietary to develop and update comprehensive care plans
Participate in interdisciplinary care conferences
Monitor regulatory compliance related to RAI processes and documentation standards
Assist with audits, surveys, and quality improvement initiatives
Provide education and guidance to staff regarding documentation requirements
Track assessment schedules and ensure submission deadlines are met
Current, active California LVN license in good standing
Experience in long-term care or skilled nursing facility required
Strong knowledge of MDS 3.0, RAI process, PDPM, and Medicare/Medicaid reimbursement
Proficiency in EHR systems (PointClickCare experience preferred)
Excellent organizational skills and attention to detail
Strong communication and interdisciplinary collaboration skills
Ability to sit, stand, and walk throughout the workday
Occasional lifting up to 25 pounds
Prolonged computer and documentation work
