At Marysville Post Acute, we are committed to providing compassionate, high-quality care in a supportive and collaborative environment. We are seeking a dedicated LVN Unit Manager to lead care coordination efforts and ensure our residents receive exceptional, personalized care.

What You’ll Do

• Serve as a key point of contact for residents and families regarding care plans, therapies (PT/OT), and treatment updates
• Coordinate and oversee admissions, discharges, and referrals
• Participate in daily stand-up meetings to support team communication and continuity of care
• Monitor resident progress and adjust care plans as conditions change
• Perform utilization reviews to ensure timely and cost-effective care
• Ensure accurate, compliant, and timely documentation
• Communicate effectively with physicians, providers, and insurance/payor sources
• Support residents and families so they feel informed, safe, and confident in their care

What We’re Looking For

• Active LVN/LPN license (RN a plus)
• Bachelor’s Degree in Nursing
• Minimum 2 years of clinical experience
• Knowledge of Medicare, Medi-Cal, and Medicaid
• Experience with PointClickCare (PCC) preferred
• Strong communication, organization, and time management skills
• Ability to work independently in a fast-paced environment
• Team player with a collaborative mindset

Physical Requirements

• Ability to stand/walk frequently and lift up to 50 lbs
