JOB RESPONSIBILITIES

  • Scheduling/Staffing- Coordinate and monitor staffing and scheduling in response to referrals. Contact home care patients/families, to schedule home visits with clinical providers. Under direction of clinical manager, and in collaboration with Clinical Coordinator team, communicate with field personnel such as nurses and therapists to ensure home visits are scheduled to clinicians with the appropriate skills to complete physician's orders. Utilize other members of the home care team as necessary to coordinate services.

  • Communication- Communicate effectively with field staff to address matters of scheduling and care coordination. Ensure optimum communication between departments.

  • Systems- Document demographic information as obtained. Ensure accuracy, appropriate documentation, resolution of issues and reschedules. Accurately pre-register patients at time of scheduling if within pre-registration time range. Collaborate with individual divisions to handle their scheduling needs.

JOB QUALIFICATIONS

  • High School Diploma

  • Vocational or continuing education in office services OR Associate's Degree in a related field

  • 2+ years related experience

  • Related health care experience OR Staffing experience
