Volunteers will be given a patient questionaire to screen patients for gaps in social determinant of health screening (transportation, food insecurity, housing instability, etc). 

The volunteer will then give this screen to the physician conducting the visit. 
Volunteers will provide outreach to patients. 

Volunteers will be given patient phone numbers (for patients lost to follow-up) to inquire if the patient would like to be scheduled in clinic. If the patient is already scheduled then the volunteer will be given the patient phone number to call in order to confirm the appointment.
