Compensation Package:

• Compensation is piece-rate, paid per completed patient visit
• Per Visit: $75 – $150 per completed visit (good-faith estimate upon hire based on type of visit)
• Mileage Reimbursement: $0.72 per mile

Job Summary

As a Physical Therapist (PT) with GrandCare East, you will provide in-home physical therapy services according to a physician’s plan of care.

Responsibilities

• Evaluate patients and home environments to assess functional status, muscle strength, mobility, gait, ROM, and rehabilitation potential; contribute to the overall plan of care.
• Deliver and direct physical therapy treatments according to the physician’s plan of care, adjusting based on patient response and communicating changes to the physician.
• Educate patients and caregivers on therapeutic exercises, safe use of assistive devices, and care of equipment; provide guidance to family and other personnel involved in care.
• Document clinical progress, prepare required summaries, participate in case conferences and discharge planning, and refer patients for additional home health services when needed.
• Supervise support staff, including Physical Therapy Assistants and (when PT is the only skilled service) evaluate and supervise home health aide care every two weeks.

Requirements

• Current California Physical Therapy License
• 2+ years of physical therapy experience (home health preferred)
• CPR certification
• Valid driver’s license and insured vehicle
