JOB SUMMARY
The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.
DUTIES & RESPONSIBILITIES
As needed, develops and initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Also Initiates proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care.
Ensures a Clinical Manager/Supervisor is available during all operating hours.
Responsible for assuring the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders, including medication orders, from all physicians involved in the plan of care.
Receives case referrals and accepts those who the agency is capable of providing services for.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Must be a registered nurse, licensed physician, physical therapist, speech-language pathologist, occupational therapist, or social worker with current licensure to practice in the State.
