Responsibilities

As the Care Plan Coordinator for Community Health Partners, you will serve as a patient advocate, liaison and educator to assist patients with their care plans. Actively working with the healthcare team and family members, you will be integral in tracking the patient's care plan to ensure treatment and services. From providing resources, to coordinating services, to reviewing care plans, there will be constant collaboration to provide timely transitions between treatment and services. Qualifications

• Bachelor's Degree in health related field required
• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate AND 5 years of experience as a care coordinator may be substituted for the Bachelor's degree and experience requirements
• 2 years of experience as a care coordinator, patient liaison and/or patient advocate required
