Job Summary:

In this role, you will provide Individualized Supported Living (ISL) services, ensuring that each client receives personalized support tailored to their unique needs. Your efforts will directly contribute to the well-being and independence of those we serve.
As a Primary Support Professional, you will work closely with the Program Coordinator to implement Individual Support Plans (ISP) effectively. Your responsibilities will include coordinating daily activities, training staff, and ensuring that clients receive the highest quality of care and support.

Key Responsibilities:
Work in the home and/or community with assigned client(s), as designated by the Program Coordinator and/or the Regional Director.
Participate in Individual Support Plan meetings and RN visits or service monitoring visits.
Train staff on each client’s medical needs, personal preferences, and compliance.
Oversee daily residential notes, goals, and health tracking.
Maintain house calendar and update any appointments.
Ensure all annual medical appointments are up to date and report outcomes to the supervisor.
Review the physical condition of the home to ensure health, safety, and cleanliness standards are met.
Complete event reports in accordance with state guidelines.
Collaborate with the Program Coordinator on follow-up to monthly RN visits and trainings.
Ensure safety drills are conducted according to the Individual Safety Plan.
Other duties as assigned.
Education, Experience, and/or Credential Qualifications:
Must be 18 years of age; must have a high school diploma or GED.
One year of experience in the field of intellectual/developmental disabilities or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.
Additional Qualifications:
Current driver’s license, acceptable driving record, and current auto insurance.
Successful completion of background checks including criminal record, driving record, abuse/neglect, and fingerprint check.
Physical Requirements:
Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.
Repetitive movements of hands, fingers, and arms for typing and/or writing during work shifts.
Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
