Job Summary:
Join our compassionate and collaborative team as a Clinic Nurse Manager where you'll have the unique opportunity to impact the lives of individuals in the Behavioral Crisis Center. In this role, you will be a vital resource to Nurse Care Managers and contribute to effective disease management education for our at-risk population. We are seeking a skilled leader who can coordinate care among multiple providers, as well as develop and implement wellness initiatives that foster community health. Your expertise will make a significant difference in patient outcomes, ensuring our clients receive the best support possible after hospitalization.

This position involves overseeing the admission and discharge processes for the Behavioral Crisis Center, while being the bridge between patients and complex health conditions including their diagnosis and treatment plans. Key Responsibilities:

  • Develop wellness and prevention initiatives to enhance patient care

  • Cooperatively develop health goals and update treatment plans post hospitalization as needed

  • Manage the process of hospital admission and discharge for Healthcare Home patients

  • Train patients on managing complex health conditions, including diagnosis, treatments, and medications

  • Act as a resource for Nurse Care Managers for complex client needs

  • Participate in required meetings, evaluations, and adhere to Brightli Operational Guidelines

Education, Experience, and/or Credential Qualifications:

  • Advanced knowledge acquired through the completion of an RN or LPN license

  • Experience with direct patient care preferred

  • Knowledge of chronic health conditions in both youth and adult populations

  • Lead or Charge Nurse experience preferred

Additional Qualifications:

  • Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check

  • Completion of New Hire Orientation at the beginning of employment

  • Current driver’s license, acceptable driving record, and current auto insurance

Physical Requirements:

  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.
  • Sedentary work involves sitting most of the time but may involve walking or standing for brief periods. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
