As a Community Health Worker, you will serve as a vital liaison between health providers, social service organizations, and the community. You will help patients overcome barriers to care, provide education on reducing health risks, and connect them with services that promote long-term wellness. Key Responsibilities:

  • Facilitate communication and empower patients in navigating healthcare and social service systems.

  • Troubleshoot barriers to care and assist patients in overcoming challenges to adherence.

  • Conduct community outreach to recruit and engage high-risk patients.

  • Serve as a liaison between health/social services and the community to improve service quality and cultural competence.

  • Assist care teams in developing care management processes, disease management, and patient education.

  • Build individual and community capacity through education, advocacy, and social support.

  • Connect patients with community resources and health center services as appropriate.

  • Provide follow-up support through phone calls, home visits, and community engagement.

  • Conduct public education to raise awareness of Qualified Health Plans (QHPs).

  • Provide impartial information and assistance with health coverage eligibility and enrollment.

  • Deliver culturally and linguistically appropriate services tailored to diverse populations.

  • Perform special projects and other duties as assigned.
     

Education and/or Experience Qualifications:

  • Bachelor’s degree in a related field preferred.

  • Two (2) years of experience in community health, human services outreach, or healthcare preferred.

  • Certification as a Community Health Worker preferred.

  • Desirable experience includes customer service, patient/peer coaching, or medical patient support.
     

Required License/Certification (one of the following):

  • Community Health Worker Certification (preferred).
     

Additional Qualifications:

  • Knowledge of harm reduction, motivational interviewing, and relevant treatment modalities.

  • Familiarity with local community culture and values.

  • Strong customer service skills, including needs assessment and satisfaction evaluation.

  • Excellent verbal and written communication skills.

  • Active listening and interpersonal skills to build trust with patients.

  • Interaction, community relations, outreach, data entry and public speaking.
