As a Community Support Specialist, you will work directly with individuals experiencing mental health and/or substance use challenges, many of whom are homeless or at risk of homelessness. In this role, you’ll help connect clients to housing, healthcare, and critical community resources—empowering them to move toward independence and long-term success.

This is a highly hands-on, community-based role—perfect for someone who enjoys building relationships, advocating for others, and making a real difference every day.

What You’ll Do

As a Community Support Specialist, you’ll serve as a key support person helping individuals navigate systems and build stable, independent lives.

Housing & Stability Support

  • Help clients find, secure, and maintain housing
  • Build and maintain relationships with landlords and housing partners
  • Assist with housing applications, resources, and financial assistance options

Case Management & Advocacy

  • Provide community-based case management to individuals in supportive housing
  • Connect clients to healthcare, employment, education, and social resources
  • Advocate on behalf of clients to ensure access to needed services

Skill Building & Recovery Support

  • Help clients build independent living skills (budgeting, hygiene, daily routines)
  • Support development of coping skills and recovery strategies
  • Provide education on mental health, substance use, and wellness

Collaboration & Care Coordination

  • Work closely with treatment teams, healthcare providers, and community partners
  • Participate in treatment planning and team meetings
  • Coordinate services to support long-term success and stability

Documentation & Follow-Up

  • Maintain accurate and timely documentation of services
  • Follow up with clients and referral sources to support engagement
  • Assist with discharge and long-term recovery planning

Crisis Support

  • Support clients during crisis situations
  • Help connect individuals to appropriate crisis resources and services

What We’re Looking For

Education & Experience

We welcome a variety of backgrounds! Preferred qualifications include:

  • Bachelor’s degree in:
    • Social Work
    • Psychology
    • Counseling
    • Criminal Justice
    • Human Services or related field

OR

  • Relevant experience working with individuals with:
    • Mental health conditions
    • Substance use disorders
    • Homeless or at-risk populations

Additional Requirements

  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to travel throughout the community to meet with clients
  • Must meet background screening requirements
  • Ability to complete required onboarding and ongoing training

Physical Requirements

This role involves a combination of community-based work and documentation, including sitting, standing, and walking for periods of time. Occasional lifting (up to 10 lbs) may be required.
