Starting pay of $42.21/hr, Pay will commensurate with experience

Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.


 

Job Description Summary:


 

Manages nurse home visitors and the community specialists. Responsible for overseeing the implementation of the Family Connects Model with performance depending on monthly key performance indicators. Technical assistance will be regularly provided by Family Connects International to help achieve these goals.

Essential Functions:

  • Immerses into the Family Connects nurse home visiting protocol and performs home visits as needed.
  • Oversees training of new staff. Establishes and coordinates a scheduling and assignment process for home visits. Includes regular visits to and contact with hospitals, clinics, and community-based organizations.
  • Implements quality assurance practices, including quarterly visits with each nurse home visitor to assess commitment to the Family Connects Model.
  • Leads continuous program improvement efforts based on program implementation data, patient feedback, and community input.
  • Leads a weekly case conference meeting for nurses and community specialists.
  • Networks with community stakeholders to promote the program, seek feedback, and collaborate on mutual goals.

Education Requirement:

  • Associates degree in Nursing, required. 
  • BSN, preferred. 

Licensure Requirement:

Active Ohio RN license, required.

Certifications:

BLS, required.

Skills:

  • Proficiency with laptops, electronic health records, and Microsoft Office applications. 
  • Ability to be flexible and creative.

Experience:

  • Community Health experience, required.
  • 2 years of relevant clinical nursing experience, such as postpartum, lactation consultant, or home visiting experience, required.
  • Experience working with diverse populations, required.

Physical Requirements:

OCCASIONALLY: Blood and/or Bodily Fluids, Communicable Diseases and/or Pathogens, Electricity, Lifting / Carrying: 11-20 lbs, Machinery, Patient Equipment, Pushing / Pulling: 0-25 lbs, Reaching above shoulder

FREQUENTLY: Bend/twist, Climb stairs/ladder, Color vision, Depth perception, Driving motor vehicles (work required) *additional testing may be required, Lifting / Carrying: 0-10 lbs, Peripheral vision, Seeing – Far/near, Squat/kneel, Standing, Walking
