A Brief Overview

Assumes responsibility for daily coordination of the resident's care, including comprehensive assessments, care conferences, and written plans of care. The RN-MDS Coordinator oversees, coordinates, & assumes responsibility for resident care which includes but is not limited to admission, discharge, and transitions of care. The MDS Coordinator assures that quarterly, annual, & significant change observations & assessments are completed in a timely fashion for assigned group of Residents. Member acts as a resident advocate, provides leadership, organization, and direction to the nursing department.

What you will do

  • Audits resident charts, care plans, and charting to ensure documentation supports the care provided and reimbursement level.

  • Completes and transmits MDS assessment and reports in accordance with regulatory requirements and facility policies and procedures.

  • Identifies and communicates need for significant change in status assessments.

  • Ensures compliance with guidelines for participation in Medicare, Medicaid and other benefit programs.

  • Maintains a viable working relationship with all disciplines involved in the planning of resident care.

  • Completes Discharge Assessment and entry/re-entry tracking forms per Federal regulations.

  • Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar.

  • Notifies members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings.

  • Electronically submits MDS information. Reviews, revises, corrects, and resubmits as appropriate;. Contributes to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care.

  • Participates in facility QA initiatives.

  • Will be required to train as part of the Infection Control team for Long Term Care.

  • Understanding and following established policy and procedures related to fire, tornado, evacuation, disaster plan, universal precautions, infection control procedures, hazardous and chemical communication program.

  • Mentor new employees and assists with their orientation.

  • Lead, by example, following Freeman Regional Health Services CARE standards.

Required Education, License/Certification, or Work Experience:

  • Registered Nurse (RN) - Board of Nursing - An active license in the state of practice is required upon hire

Preferred Education, License/Certification, or Work Experience:

  • 1-3 years Long Term Care nursing with MDS experience
