Geisinger is seeking an experienced Nurse Practitioner or Physician Assistant for a per diem position in various skilled nursing facilities as needed.Job Duties:
This position requires an experienced advanced practitioner, prior experience working in a skilled nursing facility (SNF) is helpful but not required. The SNF advanced practitioner (AP) provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.

Position Details:
Family Nurse Practitioner, Adult – Geriatric Nurse Practitioner or Physician Assistant with advanced practice experience in a skilled nursing facility or long-term care facility is preferred.
Education:
Master's Degree-Nurse Practitioner (Required)Experience:
