Assists QI/QA Director to meet all Health Resource Service Administration (HRSA), clinical requirements for the FQHC LAL. Acts as the coordinator within Community Care to identify appropriate patients and opportunities regarding scheduling, documentation and plans of care to support health management. Collaborates with patients/families/legal representatives, at multiple Community Care sites, to facilitate clinical guideline adherence and coordinates services specifically related to the patient's health care needs. Serves as a vital participant for tracking or follow up activities related to projects implemented from Federal or State grant awards for FQHCLALJob Duties:
