About the Role

As an Adult Home Care Physical Therapist, you will:

  • Perform comprehensive evaluations and assessments 🩺

  • Develop individualized treatment plans tailored to each patient’s needs āœļø

  • Deliver skilled therapy interventions to improve mobility, strength, and function šŸ’Ŗ

  • Support patients in remaining in their homes safely and independently šŸ”

Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties. Case load territory is assigned after orientation.

Minimum Qualifications āœ…

  • Bachelor’s degree or higher in Physical Therapy

  • 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance

  • Home health experience preferred, but not required. We will mentor and support you.

  • Current Vermont Physical Therapist license, or willingness to obtain one 🌱
